TRANSFERRING CLINICS
Are you moving to Hamilton or looking for a change in care?

When transferring from your current clinic to ours, we will need to have you sign a consent form. The form will allow us to receive medical records from your previous clinic, making sure we can continue the same care you were receiving as before.
This won't take long, but it does need to happen before we can book your appointment.
AFTER HOSPITAL / REHAB / JAIL
If you were recently released from a hospital, rehab facility, or jail and are looking to continue care, you just need to let our staff members know.

When you are released, we need to get some information from the institution that was handling your care. We will have you sign a consent form that will allow us to receive information about your previous care.
The information will include who was prescribing to you, what medications you were receiving, and at what dosages. This will give the doctor looking after you at our clinic a picture of how they will be able to continue your care.
This won't take long, but it does need to happen before we can book your appointment.
YOUR FIRST APPOINTMENT
Welcome!

Your first appointment will run about half an hour, where the doctor will get to know you, outline different treatment options, and decide with you what will be best suited for your needs.
You will be asked to provide a urine sample upon your first visit to help the doctor assess treatment options.
